Please print this page, fill out the form, and mail (with check payable to NWAVE Committee) by Sept. 20 to:

You may also download this form in .txt format. Save the file to your hard drive in .txt format under your last name (e.g., "smith.txt"), enter the requested information, and email the file to Sherry Ash (ash@babel.ling.upenn.edu). Please mail your check separately by Sept. 20 to the address above.

Onsite registration will be available during the conference. A $25 surcharge will apply to the basic registration fee.

International attendees: We can only accept checks in U.S. currency. Attendees who reside outside the U.S. may pay onsite at the preregistration rate. However, you must mail or email the completed preregistration form to the Treasurer by September 20 to qualify for the preregistration discount.
